I like Jessica's idea though. I have always kept the other stuff to the side and just stuck with "rules" in my contract. Makes me think though that maybe it's time to change...

ipage front and back...front has the details back has the conditions and sigs

3 - 1 page front and back is the contract. 1 page is information (times, locations, details, package, etc). and 1 page is payment schedule, receipt, etc.
